Educational Health Care Plans

 

An Education, Health and Care Plan (EHCP) is a legal document for children who need support beyond what school can normally provide. It brings together a child’s educational, health and social care needs to ensure they receive the right help to make progress.

An EHCP may be considered when a child is not making expected progress despite targeted SEN support or when long‑term, coordinated input from different professionals is needed.

 

Documents Needed for an EHCP Application

When requesting an EHCP needs assessment, schools and families are asked to provide:

  • School reports and assessments

  • SEN Support records (e.g., provision maps or support plans)

  • Professional reports (e.g., educational psychology, speech and language therapy)

  • Parent views

  • Pupil views (where appropriate)

  • Any relevant behaviour or pastoral information

Local authorities may request additional evidence depending on the child’s needs.
